The HOMES Team Program provides services to mental health consumers through the employment of the psychosocial model of rehabilitation.  Through this “mobile approach” to psych rehab, services are taken to the members’ homes and apartments, in order to pursue and accomplish self-determined goals. 

Members of the HOMES Team are mental health consumers residing in Montgomery County who meet eligibility requirements. Most often, mental health care professionals such as case managers, psychiatrists and therapists, have referred these members to our service.  Most importantly, potential members must have a legitimate need for services as well as be committed to working toward the goals of their choice.

Our members’ goals encompass a variety of areas, however all lie within one or more of five “domains”.  These domains or “areas” are: Living, Illness and Wellness, Vocational, Educational, and Social.   Examples of services provided include activities of daily living (ADL) skill training, menu planning assistance, budgeting assistance and organizational help.  The program may also support members by mediating roommate issues and assisting them in finding ways to develop their skills through education and entitlement.

Goals are pursued during meetings with a rehabilitation specialist that take place approximately once per week for a session lasting an hour or two.  Sessions generally occur Monday through Friday during business hours, however it is not uncommon that a meeting may occur outside of these general times.  These meetings most often take place in the member’s homes, apartments or HOMES Team facility based apartments.

For members of the HOMES Team who require emergency assistance outside of general business hours, there is access to our on-call system which puts the member in touch with an employee of Hedwig House. 

Members of the HOMES Team also have the opportunity to attend activities that occur on evenings and weekends.  These activities are designed to increase social skills and are intended to provide both a fun and educational experience.  Aside from the regular activities program, members may attend special events that occur seasonally and include camping trips, an annual holiday luncheon and others.  Alumni of the program are those members who have reached their goals, have been discharged from the HOMES Team and have chosen to participate in the activities program.  They also have the option of attending these activities and events.

Overall, through hard work and dedication to the program, many of our members have experienced success in their rehabilitation plans. Through these success stories it can be seen that this mobile approach to psychosocial rehabilitation allows mental health consumers to pursue and accomplish their goals.
