The HOMES Team is currently set up to accommodate 35 members.
We receive new members through a standard referral process.
Typically, it will be the caseworker, therapist, or other professional who will recognize that a consumer might benefit from a mobile psychiatric rehabilitation approach.
Gleaning information regarding the HOMES Team from our website or through some personal contact that professional would first discuss the HOMES Program with his/her client to ascertain if there is client interest in membership.
(It should be remembered that ours is a goals oriented approach in which the member will be expected to set realistic targets and endeavor to make weekly progress toward those desired ends.)
If a consumer is so oriented the professional would submit a completed referral form which must be accompanied by a current psychiatric evaluation.
When the form has been received at our office the following chain of events is set in motion:
· Upon receipt of all the required information a letter of acknowledgment will be sent to both the referring person and to the client.
· Phone contacts will be made to set up an interview date. We generally have two of our staff persons attend the interview. It is desirable to have the referring person present, as well as any other person who is significantly involved in the client’s day to day life.
· Subsequent to the interview, the entire HOMES Team staff will meet to go over the information received via the paperwork submitted and the verbal exchange at interview in order to determine whether the consumer is, indeed, appropriate to our Program. Both the referring person and the consumer will receive a letter regarding the outcome of that meeting.
· If the consumer is accepted to HOMES Team and we have membership space the person will be immediately “on board”. If our membership is full, the consumer will be put on a “wait list” for space available.
There is no specified length of time for which a consumer might be on HOMES Team membership. The span will be relevant to his/her needs and willingness to set and work toward goals within the realm of the various domains with which we deal.
Upon completion of goals, or a client choosing a stance in which he/she no longer wishes to set and move toward goals, or a client physically moving out of Montgomery County, the client will be discharged from the Program.
